To be flexible and resilient in working with care leavers from different backgrounds and experiences offering fair access to resources and support. To work confidently with diversity and understand the impact of discrimination on care leavers. To advocate, provide advice (including practical advice), guidance and support to young people in receipt of a leaving care service.To complete needs assessments, risk assessments, capacity assessments to be considered within the pathway plans. To attend, chair and manage pathway plan reviews; alongside any other meetings necessary to support transitions to full independence and adulthood. To undertake budget work with all care leavers enhancing capacity for independent living. To keep up-to-date written records of contacts with and of services provided to, the relevant or former relevant young person and ensure that these are recorded on the Integrated Children’s System within 72 hours.To liaise with partner agencies including Housing, DWP, Education, Health, Probation as may be appropriate for the young person. To ensure Housing benefit claims are made and council tax exemption applied for by relevant care leavers. 

To assist with the day-to-day running of The Cocoon and to be part of the rota, including Saturdays (9a.m. – 5p.m.To help facilitate events at the Cocoon and to encourage young adults to attend the Cocoon, as well as, events held at the Cocoon. To develop and grow the Youth Management Team and prepare care leavers to present at Corporate Parenting Panels.To identify and escalate any child or adult safeguarding concerns to the Team Manager and professional network. To participate in relevant lead areas/joint working to meet the needs of the service. This can include areas of interest (with the young people being at the centre) to help ensure improved outcomes for them.To respond to complaints and ensure all care leavers have an opportunity to have their views and experiences heard. To support with immigration matters for UASC and liaise with Home Office as appropriate. To support care leavers through bereavement and loss and use of systemic intervention
